Can't reproduce, works fine on my side.

Maybe you can set a breakpoint in modify class awfrmMB of aw method init and see why you don't get the right icon; also check xxxFrmMB.cIcon64: default in awFrmMB.cIcon64 is 'Info64.png' ('64' means the number of pixels wide).

In Bootstrap mode, you should replace these icons by FA icons using atBSicon.prg, like shown below:
